Sammy Ray Collins, 56, passed away on Sunday, February 21, 2016 at Indian Path Medical Center. A visitation will be held on Thursday, February 25, 2016 from 6-7 PM at Trinity Memorial Centers Funeral Home. A Celebration of Life will follow at 7 PM.
